The puzzle box shell is a “donor” puzzle box called “Long Life” made in China by MIToys. None of the original puzzle solution is used. 
All puzzling to open the box is the creation of Brian Young.

The text from the label: 

The puzzle is to find your way to the Command Service Module.

After showing great ingenuity the Apollo 13 crew and ground staff sorted a workaround.  

No hitting, no external tools, definitely no gaffer tape :) and definitely no force at all.

You can solve this puzzle with a real lot of dexterity. You’ll need a little for the maze part of course. But take the challenge to use the tools provided for everything else.

This puzzle would probably be a lot easier to do in weightlessness.
